Your feedback allows Yvette the opportunity to review her services and make improvements where appropriate. All feedback received will be confidential.

Feedback can be provided by using the online feedback questionnaire below. Feedback forms are also available under documents or please email Yvette here and forms can be either emailed or sent out to you. 

If you have a specific complaint you would like addressed, please complete the following form in a single session.

Yvette aims to respond to complaints within 3 days and resolve complaints within 21 working days. If Yvette can’t resolve your complaint, she’ll explain why and let you know your other options. If she needs to connect with you, it’ll be from a mobile number.
